In order to receive CE credit, participants must complete all speaker evaluation forms during the conference, complete and submit the CME Registration Form and CE Credit Request Form (available at the conference and attached below), pay a $20.00 fee, and complete all post-conference online surveys for each presentation attended.
Paper Statements of Credits are no longer issued. All CPE credit is uploaded to the NABP CPE Monitor System. Participants requesting CPE credits will be required to provide their NABP e-Profile ID number (6 – 10 digit number) and date of birth (month and day only – MM/DD). If a participant does not have a NABP number he or she will need to request a number by registering online at www.mycpemonitor.net.
